Premium rates are established for each cultural group based on its composition (total number of active members, ratio of males to females and their ages). The census form that follows may be used to obtain rate quotes for your group, as well as to apply for a group term life policy.
To obtain rate quotes, list all active members and the date of birth
and gender for each of them. You may omit social security numbers and ignore the “Children’s Rider” column and the “Applying for Coverage (Yes/NO)” column. Also, you may enter approximate ages if dates of birth are not readily available. (Be aware that, if approximate ages are used to obtain rate quotes, those rates may change when your members actually apply for coverage and furnish the required dates of birth.)
Within 48 hours of submitting the census form, you will receive rate quotes for each benefit level -- $10,000, $15,000, $20,000 and $25,000. Your group then must choose one of these levels, which will apply to each member seeking coverage.
When you are ready to apply for your group policy, all the information requested on the census form must be provided for the members that wish to be insured. U.S. citizenship is not a requirement, but applicants must have social security numbers and live in the United States or Canada.
Once you have submitted a complete and accurate census, your group policy can take effect as soon as the first day of the following month.
